Defining a COMMONLY AGREED Plan


In 1983 a Korean Airlines jet strayed deep into Soviet airspace and was shot down by Russian fighters. Was there anything wrong with the autopilot? No. Was there anything wrong with the plane itself? No.

Subsequent analysis showed that the wrong coordinates had been entered into the autopilot at the outset of the flight, and the crew had never checked their progress manually.  The pilots were absolutely sure that they had done the right thing and died for their belief.
